Date and place of birth; childs forename(s); sex of child; father's name; mother's name and maiden name (usually shown as "formerly nnnn"); fathers occupation; signature, description (relationship) and address of informant; date registered and signature of Registrar.

The registration would be under the father's surname unless he isn't named on the cert then it would be under the mother's name.

This is for English and Welsh certs - I believe Scottish ones have more info.

Can anyone tell me what information is on a birth certificate, e.g, parents names, parents dates of birth? etc. Thankyou.
