As a married woman any children would be normally registered with that surname, regardless of actual parentage.
If the birth father was present at the registration, ...or a declaration was made laer, the child's birth would also be registered with his surname.
As the details apply to the same birth, the numbers are the same.

When you say the same reference number, are you talking about the number on the actual certificate or is it a page No? Is it a birth, marriage or death cert?

if the child is illegitimate but the father acknowledges he is the father and his name is on the cert .then the reg will appear with both surnames but the same reg number. means whichever surname you order the cert in you will get the one cert

My gg grandmothers death was registered twice. Once with the surname of my gg grandfather and again with the surname of her first husband. Presume she had been "living over the brush" with my gg grandfather and the family did not find out until after her death. It was registered by the son from her "first" marriage. Reference numbers the same.

There is a very strong possibility the parents wasnt married at the time of registration,,if this is the case then the birth is registered in both surnames,,as is now done today,,,,
