Name and Surname of the Deceased
Informant's Details
Date of Registration

Name and surname
Sex
Age
Occupation
Cause of death
Signature, description and residence of informant
When registred
Signatuire of registrar

As prevoius poster said it can vary.

also remember that some of the information on a death certificate, no matter WHEN it was issued, is actually only hearsay ........... ie, what the informant knew or was told by the dead person


This "hearsay" information includes

name of deceased
age of deceased
marital status of deceased (if shown)
occupation


The accurate information will be

date of death
place of death
cause of death
sex of deceased
name and address of informant
relationship of informant
name of doctor
name of coroner and date of inquest
