	Army Personnel Centre HQ Secretariat Historical Disclosures Mail Point 400 Kentigern House 65 Brown Street Glasgow G2 8EX Tel: 0845 600 9663 ring them up and ask for a form to be sent. It is £30 unless the widow applies. My mum is still alive,so she signed the form and we applied in her name,so they were free. 	Hi Bonnie, Children can apply as well but only the widow can get them for free.If you go onto the website that Linda mentions you can print off the order form plus a next of kin form.My husband applied for his fathers ,it's just a case of stating what your relationship is to the person whose records you want, Sue
