How do you add adopted children to "Family Tree"

I would only have them in the tree once,with their birth names and parents.
The adoption and the name change I would put in the notes

Hi:: The children in question had a mother and father. The father died and she married her second husband. The second husband legally adopted them and they took his name. I have them listed under the first father with his last name and the last name of the 2nd. father. Should this be left that way or can you put them under both fathers. Hope this explains better what I am trying to do.

I have a 2nd. marriage where the husband adopted his 2nd wife's children and they changed their last name to his. Her first husband passed away. I have put them under the original mother and father's name but also showing the new last name.
My question is should they also be shown under his name with the 2nd wife and how do you do it? I don'w want them shown with him and his first wife.
