From the book "Ancestry Trails":

Certificates:

A birth certificate should show the following:

1. Date and place of birth (plus time if twins)
2. Name of child
3. Name and occupation of father
4. Name and maiden name (or previous married name) of mother
5. Signature (or mark), name and address of the informant (usually a parent)
