All names should be maiden/birth names.
Add her first husband as well and it will all make sense.

I have just discovered that my G Grandfather remarried about 10 years after his first wife died. This new lady was also a widow. When I enter her on to my tree......do I put her in her maiden name or 1st married name? He remarried a few months after the 1911 census, hence the reason we never knew this.
