Registration of Grandmother's death

Hi everyone,
I cannot locate the registration of my grandmother's death, Ellen Agnes, maiden name Draper, married name skinner. I know for sure she died in Hammersmith/Ducane road hospital in 1960-61. Have found her birth registration and her marriage cert. As she died in hospital I can't imagine why it would not have been registered?

What about this - not sure if Willesden would cover Hammersmith, but deaths are not necessarily registered where they happened:

Not necessarily, Von, this is from the directgov website:

Registering a death in England and Wales
Where and when to register a death
In England and Wales, you normally need to register the death within five days. It's best to go to the registry office in the area in which the person died, otherwise it may take longer to get the necessary documents and this could delay the funeral arrangements.

If you register a birth or death in a different district,to the event, then that registration will be forwarded to the correct district.hence the warning of delay.

Likewise, rellie died in Kent, death registered in Devon, some 250 miles distant.
The death was registered in Devon and comes up in a search as registered in Devon, it was not sent back to Kent.

I did check out the information before I posted but should have put the sight where I found the information.
However there are obviously anomalies.
Found this on http://www.nafd.org.uk/funeral-advice/funeral-arrangements/frequently-asked-questions.aspx
"In England and Wales, the death has to be registered at the registrar's office in the area where the death occurred. This is the case even if the death occurred a distance from home.

However there is a facility available to attend your local registrar's office to register a death that occurred in another area. This is called 'Registration by Declaration', and involves the two Registrars transferring documents by fax and post in order to register the death."
Maybe things have changed over the years.

It's also worth remembering that a death certificate may not be issued immediately if a coroner is involved. Apparently a coroner might be involved if the person died after an operation or their death might be related to their occupation.
I know that if the police are involved and there is an ongoing investigation then there may be a long delay before a certificate is issued.

So, going back to the original question, the death that Potty found could well be the one....!
