Irish Death Cert - Help Please with Ordering

I want to to order a death cert from Ireland. I have the year, district, volume and page number and have downloaded the order form from the Irish GRO but cannot answer in full some of the required questions, ie :

exact Date of Death
Civil Status (no idea what this means)

Has anybody else ordered such a cert without this info and has it caused problems?

I have ordered with just the volume number etc. It was quite quick. Probably only worth ordering the for information one rather than the entire certificate. The certificate is only needed for official stuff. It is only the information on the certificate you need.

Thanks, Barbara. Did you just add the volume No, etc as a note as there doesn't seem to a place on the form for that info.

Yes, I was only going to order the photo copy one.
