What information would be written on a birth certificate issued in London during 1864, please?

If England and Wales

Full name
Date and place of birth
Sex
Mothers name and maiden name
Fathers name and occupation if known
Name of informant.

Scottish ones have far more info.

The forms for BMD certs were unchanged 1837q3-1969q1.
There were (I imagine) internal GRO directives to ensure uniformity of content.
I think unwed fathers were first allowed to be named in 1875.
