Death 'between ship and shore'

My husband's great-grandmother EMILY NEWTON (born 1 Dec 1850 - maiden name also Newton) died probably in the 1920s when my mother-in-law was a child. She was on her way (?) to visit her eldest child Cecil in Ireland (no idea where). Apparently she 'fell between ship and shore' and died. I don't know whether it was instantaneous or not. She would have been in here 70s at least. We don't know what port - Liverpool, Fishguard or wherever. I would like to find her death certificate but I haven't had any luck so far. I expect her death might be recorded in a newspaper. Any tips how I can locate her death record? It's been frustrating me for about 5 years now.

No, afraid not. Emily's husband (George Herbert Newton) died in 1885. Cecil was born in 1878, then Rose Elsie in 1879, then Percy Gerald in 1880. Percy Gerald is my husband's great-grandfather.
