I would imagine that the John Kemp, grandson aged 6, is the son of the married son John who is 47 years old. The nephew could belong to either a brother or sister of Joseph Kemp or a brother or sister of his wife. I'm not certain but I think Sarah might be the wife of John aged 47 (there are two little marks next to 'Daughter' which might mean 'daughter in law') and therefore mother to John aged 6 (this might not be right but buying the birth certificate for the 6 year old would tell you his parents names).
