I'll check again if its an off-shot although it appears to be coming from the roots.
I've been using a spray fertiliser that you use on the leaves & roots. There might be a bottle of feed left over from the last unsuccessful attempt to keep one alive!

Sounds like a dendrobium nobile. If the 'sideshoot' has roots and green growth it is a kiki and can be removed and potted into a new plant.
Liquid fertilisre weekly, weakly, until about early October, then water only until about end of December. With-hold water until the new growths start to form a lobster claw - they will be flowers. Too much water will result in kikis.

I just chop my Orchids right down once all flowers have died off . They look dead but after a few months things begin to grow and I have wonderful blooms again :-D. When I remember I add some Orchid food to water and give a feed. But just generally leave things to nature and it works :-D :-D

I've grown Phalaenopsis for years, but not had a Dendrobium for a long time.

From what I remember ............ leave the spears, let them do their own thing and only cut back when they have obviously died off.

Mine flowers better than my sister's - she takes great care of hers.
I occasionally water mine.
Sometimes I support the flower heads, sometime I don't - it still flowers :-D

Mine's a Phaloenopsis, and is like the ones you see in Garden Centres, with flower shoots that grow up and then horizontally with flowers on them. It came with very limited instructions, so I found a very ancient gardening book which had a whole chapter on orchids, from the days when Victorian gardeners grew them in hothouses!

It did come with instructions on care, but not what to do with it once it's flowered.
As they've since been lost, I don't know what it's called.

It's definitely not like the sort you've described. I think the correct term for these stalks is Spears. Before it bloomed it looked like 2 green bamboo canes with leaves coming out of those at about every 3ins. The blooms came off the spears between the leaves.

If it behaves like my orchid, the original shoots will die back and then you cut them back to a few inches. If the new one is growing upwards, it is a flower stem - if downwards it is a root. (They come from the same place!!)

Mine doesn't look like yours. The shoots and leaves all come from soil level. I know there are different types. Did yours not come with any instructions?

Anyone know how to care for them?

I have one that looks like a bamboo stalk ( see avatar)
The blooms died off a while ago & a new shoot has formed from the roots.
Do I cut off one ( or both ) of the original 2 stalks even though they are green or allow all 3 to do their own thing?
